Our photo/video consent forms are a contract for any parties whom you wish to partake in your photo and film shoot. This outlines the basic expectations from both parties and will best protect you as the business owner or project leader from any potential future concerns. If there are special requirements please advise before signing and we will do our best to come up with a reasonable solution. Please download and have completed by all parties and have returned to MoMac before filming starts. Scan and email back to admin@momac.co.nz
